You can download the claim form from our website, or we can fax or mail a copy to you. For a pet’s first claim, we need the pet’s complete medical records from both current and previous veterinary or emergency clinics. Both you and your veterinarian will need to complete our one page claim form. The form is then faxed or mailed to Direct Pet Health with a copy of your receipt. If the claim form is incomplete it will be returned to you for additional information. We may request a full history from your veterinarian(s). Adding additional pet insurance coverage to your policy is easy and perhaps the best way of ensuring you have the money you need to provide the best care to your pet. With Direct Pet Health, you can currently add Hip Dysplasia to your policy. To do so, simply call 1-800-569-7913 and one of our member specialists will be happy to assist.
You may elect to cancel your Direct Pet Health pet insurance policy at any time by giving notice to Direct Pet Health by calling 1-800-569-7913. You may be required to return the original policy or sign a "policy release", and of course you will be responsible for any premium earned through the date of cancellation.
